How do I enable media upload for specific forms only? I don't want upload on all comment forms but for specific forms or posts only.

The add-on has the "Post types supports uploading" option, so you can have two comment forms and configure the option as necessary.

It is just posts or pages in general. I don't see where I can specify upload by specific post or comment form.

Sorry for the late response.
We're really sorry, but there is no way to specify upload by specific post or comment form. Currently, the add-on supports the post types uploading. However, we've discussed it with our team and added to our to-do list. We'll add the option in near future releases.
