Hi
I have some similar problem i visit a topic where i knew some member had uploaded an image but it is gone if i edit i see there addon code and saving does not fix.
I have Wordfence security plugin but not sure if this is cause because it was working fine with it.

Just let your users to not delete their images and other attached files. We'll add a new option in next version to disable other users to delete their attached images.

Thank you Tom it seems photo is deleted i have look in that user folder and is gone.
