Hi, When I upload a picture in a post, it shows the picture in comment but doesn't attached the picture to that post when you see from media library. Is it possible to make it auto attached to that particular post? I attached a screenshot to make my question clear.
Also I want all comments to be approved manually. Now if one comment is approved for a user, his/her other comments are approved automatically. I want to stop it, where can i find the option?

I'm sorry but there is no way to attach comment attachments to posts. They are different content types.

You should use native WordPress comment settings located in Dashboard > Settings > Discussions admin page. Please read this documentation: https://wpdiscuz.com/docs/wpdiscuz-documentation/discussion-settings/before-a-comment-appears/
