I recently purchased the media uploader add-on. Uploaded it via Wordpress plug-ins and activated using my code.
I've cleared my caches and checked that it resides in my plug-in directory.
Unfortunately, the media icons do not appear in my comments section on my posts.
Any help would be appreciated.

I think something wrong with comment form. please make sure you don't use Jetpack Comments, if so deactivate it and delete cache.
Also please navigate to Dashboard > Comments > Form admin page, edit the Default Form, save it and delete cache again.
