So I am trying to get my form to look like others that I have seen. Here is a screenshot of what I am looking for? Do I need to purchase add on's to do this?

This is wpDiscuz comment form, without any add-ons. You just need to install and activate wpDiscuz. Once the installation is complete, wpDiscuz automatically replaces post and page comment forms with its own modern and AJAX powered comment form.
