This is a plugin conflict for sure. Please deactivate the plugins one by one and check after each deactivation to find a problem maker.
There is no option for the moderation team to see which items are pending approval. Would you be able to suggest me how I can offer my frontend moderation team the ability to see which posts are unapproved and which are approved.
wpDiscuz Frontend Moderation addon allows Admins and Editors to moderate comments on the front-end. It doesn't have a user role management system.

You've enabled the comments order "Most voted" by default, So once you've refreshed the page it's gone. Just disable the option and it should work fine. Another question: when I submit a comment using WPDiscuz, it's not getting added to the activity stream - only upvotes... (BuddyPress integration is activated).
Please make sure the "Allow activity stream commenting on posts and comments" BuddyPress option is enabled.
The "Allow activity stream commenting on posts and comments" BuddyPress option is located in the Dashboard > Settings > BuddyPress > Options admin page.
