Rating and Text is removed from comment replies, but how do I hack it so all form elements are removed from the reply form (radio button, check boxes, etc.)?

Just disable the according option in form settings:

That option is not available for radio buttons, check boxes, etc.

For some reason it didn't show up until after I saved it, but I see them now.
