Comments, sorting options, voting options are all appearing. However, when a user leaves a comment, there's no box to check in order to be notified of replies or new comments.

Hmm, I don't see why you've enabled Jetpack comments with wpDiscuz. As you see this is Jetpack Comment form and this affects all wpDiscuz functions. Please disable Jetpack comments and delete all caches.
