Is there a way (or a hack) to allow them to leave the comment form empty?

You can set all field not-required except the comment textarea. There is no way to submit an empty comment.
The comment would not be empty. It would either have an image uploaded or a comment form element filled out.
I mean the main comment textarea must not be empty, it doesn't matter you attach a file or fill custom fields. If the textarea is empty then comment is empty.

It would be great if we could remove the main textbox via css and build a form using custom fields.

I'm sorry but there is no way to do that at the moment. You should find some developer to do this customization. And yes, we'll add this in our to-do list and make it possible to submit comments with empty textarea, But I can't provide any ETA yet.
