I am not sure how to adjust the settings to make it work. Here some issues:

  1. It shows 2 comment areas that are required (I need only one)
  2. After submitting the comment I get an error about the captcha, but there is no captcha there anymore (only when subscribing)
  3. Here is the website: voiceandnation.com

I don't see the wpDiscuz Comment Form anywhere. Have you disabled it?

Please navigate to Dashboard > Comments > Forms admin page, edit the Default form and make sure you haven't created any additional textarea field.
