Could you please disable active plugins one by one and check it after each deactivation? Maybe we have a plugin conflict, this is the best way to detect the conflict point.

The only active plugins (except wpForo and your addon) are contact form and revolution slider (both cannot be disabled), maybe a theme conflict?

Yes, it maybe, I'd recommend change theme to the default.
