this is a conflict with other plugins or with the active theme. Please deactivate other plugins, switch to the default theme and check it. If it works, activate plugins one by one and find the conflict maker plugin. Please let us know the result.

I disabled all plugins and it doesn't work.

Is there any option to show all the fields from the beginning???

Please navigate to Dashboard > Comments > Settings > Styling admin page put this CSS code in "Custom CSS Code" textarea, save it, delete all caches and check it.
