As far as you're using the native WordPress pagination type, the comments awaiting for moderation disappear. If you uncheck the checkbox at the beginning of “Break comments into pages with…” option it'll work properly. The option is located in the Dashboard > Settings > Discussion admin page.

No. it's a mistake and the inactive one can be removed. 

By the way the problem cant' be from this. Because duplication of  plugin occured a few hours ago. But we have this problem for months.
